The name Santa Maria in Cosmedin comes from the greek word kosmidion (ornament), because of the exquisite decor inside, commissioned by Pope Adrian I.

The church was built in the sixth century over the Ara Maxima of Hercules and the Annona (a place in Roman times where was distributed free grain). Some remains of them may still be seen. It is divided into three naves separated by pillars and eighteen columns of various origins.
